The four pillars at their evidence level

What the published literature and the trial registries actually show for each direction BIOGENTHER states — compiled 2026-09-10 by Panacea Bio Chem. Evidence tiers are kept separate throughout: an animal finding is not a human finding, a recruiting trial is not an available treatment, and a biomarker endpoint is not an outcome.

Myostatin inhibition

• Follistatin (a myostatin antagonist) gene transfer has been tested in people in two published early-phase trials: in Becker muscular dystrophy (Mendell JR et al., Mol Ther, 2015) and in sporadic inclusion body myositis (Mendell JR et al., Mol Ther, 2017). Tier: INTERVENTIONAL_HUMAN, early phase, small numbers, open label.
• The wider myostatin-pathway drug class has a difficult clinical record — "many drugs targeting this pathway failed during clinical trials" (Rodgers BD, Ward CW, Endocr Rev, 2022, REVIEW).
• No myostatin or follistatin gene therapy is approved anywhere.

Heart health

• The landmark test of cardiac gene therapy was CUPID 2: a randomised, double-blind, placebo-controlled phase 2b trial of AAV1/SERCA2a in 250 patients with advanced heart failure (Greenberg B et al., Lancet, 2016; registry record NCT01643330, COMPLETED). It did not improve outcomes. Tier: INTERVENTIONAL_HUMAN, negative result.
• The high-dose follow-on study (NCT02346422) was TERMINATED in 2015 after 9 patients.
• The idea is still being pursued: two SERCA2a-modulating programmes are currently RECRUITING (NCT04703842 and NCT06061549, Sardocor Corp.). Recruiting means under study, not available. A 2026 retrospective by a principal of the SERCA2a programme reviews the field (Hajjar RJ, Can J Cardiol, 2026, REVIEW).

Age reversal

• Partial epigenetic reprogramming has produced rejuvenation effects in mice (Ocampo A et al., Cell, 2016; Lu Y et al., Nature, 2020; Browder KC et al., Nat Aging, 2022). Tier: ANIMAL.
• The only registry-confirmed human study of a partial-reprogramming therapy is a Phase 1 safety trial, delivered as gene therapy: NCT07290244 (Life Biosciences Inc., ER-100, RECRUITING, 18 participants estimated) in two optic neuropathies, with adverse events and liver-function tests among its primary outcomes. It is a safety study in specific diseases, not an anti-ageing treatment.
• Harms are an active research question, not a settled one: oxidative stress driving liver failure during in vivo partial reprogramming was reported in 2026 (Eom HJ et al., Mol Cells, 2026, ANIMAL).

Elevating human potential

• No gene therapy is approved anywhere for enhancement in healthy people.
• Registry-confirmed activity exists at the earliest phase: NCT07285629 (combined klotho and follistatin plasmid gene therapy, EARLY_PHASE1, COMPLETED 2026-04, 14 participants, serum-biomarker and adverse-event endpoints) and NCT07443826 (AAV9-follistatin with VEGF plasmid, Phase 1/2, RECRUITING, 12 participants estimated, single site in Coxen Hole, Honduras).
• Both measure safety and biomarkers. Neither reports an efficacy outcome, and a completed early-phase study is not evidence of effect.

The safety record, honestly

What has gone wrong in gene therapy, and what the field does about it — the primary literature, not a summary of it.

• The founding cases are in the primary literature: a fatal systemic inflammatory response after adenoviral gene transfer in a patient with ornithine transcarbamylase deficiency (Raper SE et al., Mol Genet Metab, 2003, CASE), and LMO2-associated clonal T-cell proliferation after gene therapy for SCID-X1 (Hacein-Bey-Abina S et al., Science, 2003, CASE) — the event that reshaped how integrating vectors are designed.
• The record is not only history. 2026 publications include a report of patient mortality after delandistrogene moxeparvovec with expert-committee considerations for mitigation (Shieh PB et al., Hum Gene Ther, 2026, CASE) and a systematic review of immune-mediated myocardial injury and myocarditis after gene replacement therapy (Maurizi N et al., Circ Heart Fail, 2026, REVIEW).
• The field's response is active management: sirolimus for steroid-refractory hepatotoxicity after AAV gene therapy in Duchenne muscular dystrophy (Leon-Astudillo C et al., J Neuromuscul Dis, 2026), a prophylactic sirolimus protocol for delandistrogene moxeparvovec-rokl (Siddiqui Z et al., Hum Gene Ther, 2026), mechanistic work showing hepatotoxicity via cytokine and macrophage activation (Sakai A et al., Liver Int, 2026), determinants of hepatic enzyme elevation after onasemnogene abeparvovec (Mundada V et al., J Neuromuscul Dis, 2026), and cardiac safety outcomes with up to 5 years of follow-up (Veerapandiyan A et al., Cardiol Ther, 2026).
• The honest summary: risk in gene therapy is managed product by product and dose by dose, with immunosuppression protocols. It is not a property that can be claimed away in general.
